Default Left vs. mono output in professional audio equipment

Hi all...

Just trying to work through a small problem and I'm wondering if
anyone can help. I have a pair of headphones with a stereo 1/4" jack
on them. When I plug them into certain mixers, I can hear a distinct
left and right mix in each ear.

The other day I was going through a storeroom at work trying to
determine if a bunch of gear is functional or not. I started by
testing three graphic eq's and an old mixer. In each case, when I
plugged my headphones in to the one main output on each, I only heard
output in the left side. In the case of the mixer, I was able to hear
output in both ears when I removed the plug by one "click". So, my
question is: Does this mean that these devices are outputting in mono
and they are only coming through on one side because I am using stereo
headphones? Or is there something wrong with them?
