Do you have vcr or tv tuner that's connected to CATV running into the
pre-amp ? I had a similar problem and ended up connecting 2 75ohm
baluns back to back inline with the cable.

Keep in mind that most 75300 ohm "transformers" are actually
autotransformers do not have isolation between input and output.
