Does anyone do a *real* optical system for long(ish) distances eg up to
100m? Not TOSLINK plastic cable.

Several folks do. The cheapest is probably the Hosa gadget that allows you
to run S-PDIF over a single 62.5 fibre. For more money you can get gadgets
like the LightViper that allow you to run multiple channels over the same
fibre.

Ear makes a device that converts adat to cat5 and back I use that for
long distances,

CAT5 is probably OK but optical sounds better for audiofools.

Plus, of course, there is no possibility whatsoever of earth loops
screwing things up.

If you're using unshielded Cat-5 and the devices use transformer isolation
(as is required in the AES/EBU spec but not the newer IEC spec), then there
is no possibility of ground loops anyway.

True, but optical always seems 'cleaner' IMO, although it does come with
its own problems.
