Deciding between an Alpine CDM-9821 (16Wx4) and a CDM-9823 (18Wx4). Does
the 2 extra watts really make a difference? I don't need any preamp
outputs... So is the extra $40 worth the "better clarity and more power"?
This will be replacing the stock player in a '99 Civic plus I want the AUX
to plug in my iPod.

No. You will not notice any difference whatsoever in those power ratings.
Although the $40 may be worth it for some other features, but only you can
decide that.

The wattage difference isn't significant. The stock head unit and
speakers are rated for about 16W.
Why no line-out jacks? Get them. You'll get the urge to add an
amplifier and better speakers sooner or later. A simple 60W x 4 amp
under the seat and some quality speakers in the factory mounts makes a
world of a difference.
