Big ol' 3 space Passive EQ

recently inhereted a big damn Cut-only passive EQ with a detached green face
dust cover that stated the brand name (WHich I forget here at 4am)

Here's the problem, the input and output are barrier strips, and I want to
wire this thing to use as an insert...
Input and output are the same, a five position strip with the first spot
being a ground and then the on the next four spots, there's a jumper between
2&3 and that matches with a diagram saying "600" above the strip/ it also
has a diagram for "150" which fatures jumpers between 1&2 and then again at
3&4.

I'm guessing those are impedance values, and I'm alos guessing I want the
600, the higher value for board insert use.

is this thing a balanced device? The ground is labeled clearly, where does
the hot positive go?
