Go to the Mackie web page and download the manual for the 1604 mixer.
It has several hookup diagrams including a couple for live sound
applications. They hook up the same way regardless of who makes them.

Sure it has a monitor out. That's what auxiliary sends are for. Use a
pre-fader one for your foldback mix. You could use one pair of
subgroups to feed the PA system and another pair to feed a stereo
recorder, or insert or direct outputs to feed a multitirack recorder.
Understand that there may be some things that you'll want to send to
the recorder that you don't want in the PA mix (or don't want much of,
like amplifiers, if it's a small venue). By using separate subgroup
busses, you can assign some channels to the recording mix that you're
not using in the PA mix, and vice versa (like CDs played during
intermission).
