Default Pro Tools levels - what's going on?

I needed to to make slight adjustments to the speed and pitch of a
badly-recorded acoustic '78' (which was actually wavering from 80 to
about 85 rpm), so I imported the 16-bit mono file into Pro-Tools 5.01 to
use the pitch-changing plug-ins.

The original file had about 2db headroom, but when I Bounced it back, it
was over-modulated and had horrible clipping.

As a check, I recorded a 1Kc/s tone at 10dB below peak, imported it into
a 16-bit Pro-Tools session and then Bounced it straight back again
without altering it in any way. The result was 3.56 dB louder.

What is going on?

It was a single mono AIFF track and I exported it in mono..

I had previously suspected that something like this was happening a long
while ago, but then I was using multiple tracks so I assumed it was due
to the tracks adding. This time there were no other tracks and I made
no changes to the single track at all, it should have come back out
bit-for-bit accurate.
