I ran into this problem for an iSkin case I bought for my iPod. In this
case the hole provided was circular but with a diameter too small for my
Etymotic 6s. So I took a drill bit that was a little bigger than the
hole, I turned the bit by hand so to shave through a bigger diameter. I
turned the bit by hand because I was afraid I'd crack the plastic if I
turned it with the motor. It worked great, didn't take long, and it
looks good too. I suppose this could be done through a slot, but you
might have a hard time keeping the bit centered.

That's a very creative answer. It would still need a straight and not an angled plug.
