You have a very detailed and complex question, but just for starters:

S/PDIF carries two audio channels

It appears to have three S/PDIF ports, so that means it can carry three
discrete digital channels. But for typical DVD sound, the 6 channels are
encoded in a single S/PDIF stream using something like Dolby Digital.

Before you waste too much time trying to make a breakout cable, do you
have either an old enough operating system or a new enough driver so
that your computer will recognize the card at all?
